What Step 2 CK measures

USMLE Step 2 Clinical Knowledge tests whether you can apply medical knowledge and clinical science to patient care under supervision. It emphasizes diagnosis, management, health promotion, disease prevention, ethics, quality, and patient safety across clinical disciplines.

Vignettes ask you to combine history, examination, investigations, and context to choose the best next action. Strong preparation develops clinical reasoning as well as recall.

02

Eligibility

Eligibility categories generally match Step 1: eligible students or graduates of LCME-accredited MD programs, COCA-accredited DO programs, or eligible medical schools outside the United States that meet ECFMG requirements. Confirm your status in the current Bulletin before applying.

03

Format and test day

For administrations using the testing software introduced on May 7, 2026, Step 2 CK is a one-day, approximately nine-hour exam.

ElementWhat to expect
Question typeSingle-best-answer clinical vignettes: diagnosis, next best step, management, prevention, and ethics
BlocksSixteen 30-minute blocks
Total questionsUp to 318
Test day lengthAbout 9 hours, including the tutorial and break time
Unscored itemsSome pretest questions are included and not scored

Content spans internal medicine, surgery, pediatrics, obstetrics and gynecology, psychiatry, neurology, and preventive medicine, with systems-based practice, patient safety, and legal and ethical issues woven throughout.

04

Scoring and results

Step 2 CK is reported on a three-digit scale with a pass/fail outcome. For exams taken on or after July 1, 2025, the minimum passing score is 218 (it was 214 before). USMLE reviews passing standards about every four years and announces changes in advance.

Results usually arrive within four weeks; allow at least eight weeks when planning around ERAS or other deadlines. Your score report also includes a performance profile by discipline and system.

Score scale and ranges

Scores are reported on a three-digit scale that runs up to 300. In practice, nearly all examinees score well inside that range. USMLE publishes a reference-group mean and standard deviation each year: for first-time takers from U.S. MD programs in 2024–25 the mean was about 250 with a standard deviation of about 15. The standard error of the estimate is roughly 8 points, so two scores a few points apart are not meaningfully different.

Approximate bands based on the published mean (≈250) and SD (≈15)
ScoreHow it compares
Below 218Fail
218–234Pass; more than one standard deviation below the reference mean
235–264Within about one standard deviation of the mean (where most examinees land)
265 and aboveMore than one standard deviation above the mean

These bands are a rough guide, not official percentiles. Check the current USMLE Score Interpretation Guidelines for the latest reference figures.

05

How to apply

U.S. MD and DO students and graduates apply through NBME/MyUSMLE. Eligible students and graduates of medical schools outside the United States apply through FSMB. ECFMG Certification still has separate requirements for international medical graduates.

06

Build a preparation plan

Build study around common presentations and “next best step” decisions. Mix medicine, surgery, pediatrics, obstetrics and gynecology, psychiatry, preventive care, and ethics. An error log should record the decision point you missed, not just the diagnosis.

  1. Organize review around common presentations and next-best-step decisions.
  2. Practice mixed clinical vignettes alongside clerkship learning.
  3. Keep a concise error log: knowledge gap, reasoning, reading, or timing.
  4. Include ethics, quality improvement, prevention, and patient safety.
  5. Use official sample questions and the testing interface for your exam date.
