Suggested timeline

Before clerkships end

Use clerkship shelf exams and question banks as ongoing Step 2 CK preparation.

6–8 weeks out

Dedicated review by specialty with daily mixed question blocks.

3–4 weeks out

Questions-heavy phase and your first self-assessment.

1–2 weeks out

Final self-assessment, error log review and logistics.

Last 2 days

Light review and rest.

Eligibility and registration

• Read the current USMLE Bulletin of Information end to end
• Confirm you meet the eligibility rules for this Step (school status, graduation, ECFMG certification for IMGs where it applies)
• Create or check your account with the registration organization (NBME for U.S. students, ECFMG/Intealth for IMGs)
• Make sure your legal name matches your government ID exactly
• Apply for the exam and keep your application confirmation
• Request test accommodations early if you need them; approval can take months
• Schedule your exam within your eligibility period and keep your Scheduling Permit
• Choose a test center and book early for your preferred date; reschedule only within the published deadlines

Plan your preparation

• Review the official USMLE Step 2 CK content outline and blueprint
• Decide whether to test right after core clerkships or after a dedicated study period
• Check what your target residency programs expect and any deadlines for score reporting
• Pick a start date, test date and weekly hour target; add buffer weeks
• Choose one question bank and one core review resource
• Take a baseline practice block and note weak specialties

Clinical content review

• Internal medicine: cardiology, pulmonology, GI, nephrology, endocrinology, hematology/oncology, infectious disease, rheumatology
• Surgery: trauma, acute abdomen, perioperative care, vascular, breast and endocrine surgery
• Pediatrics: development, vaccines, congenital disease, neonatal care, common childhood illness
• Obstetrics and gynecology: prenatal care, labor complications, contraception, gynecologic oncology
• Psychiatry: mood, psychotic, anxiety, substance, personality and childhood disorders
• Neurology: stroke, seizure, headache, neurodegenerative and neuromuscular disease
• Emergency medicine and critical care: resuscitation, shock, toxicology, environmental injury
• Dermatology, ENT, ophthalmology, orthopedics and urology
• Preventive medicine, screening and vaccination guidelines
• Biostatistics, epidemiology and evidence-based medicine
• Ethics, law, communication, professionalism and patient safety

Clinical reasoning skills

• Practice the pattern: diagnosis, best next step, then most appropriate management
• Learn first-line treatments and when to escalate
• Learn when imaging or labs are indicated and the order you should use them
• Review common drug interactions, adverse effects and contraindications
• Practice interpreting ECGs, imaging, labs, and clinical images

Practice questions and self-assessments

• Choose one primary question bank and finish it rather than sampling many
• Do timed, random, mixed blocks every study day once content review is underway
• Review every explanation, including questions you got right
• Keep an error log: concept missed, why you missed it, the correct rule
• Work through the official USMLE sample questions and practice materials under test conditions
• Take an NBME self-assessment (or equivalent) early to find gaps and another near the end to check readiness
• Review self-assessments in detail, not just the score
• Practice questions that combine several specialties in one vignette

Wellbeing and habits

• Study in focused blocks with short breaks; protect sleep over late-night cramming
• Exercise and get outdoors a few times a week
• Set a weekly rest day or half-day
• Tell a friend, mentor or advisor if you are struggling; ask for help early
• Review your plan weekly and adjust it without guilt

Final two weeks

• Confirm your test center address, travel time, parking and the exact start time
• Re-check your ID name against your Scheduling Permit
• Do one full practice day at the same time of day as your exam
• Review your error log, high-yield notes and flashcards instead of new material
• Plan test-day food, water and comfortable layers
• Lock in sleep and cut back on anything that disrupts it
• Prepare a short list of go-to calming techniques for a hard block

Test day

• Arrive early with the required ID and your Scheduling Permit
• Complete the tutorial quickly and save time for the exam
• Pace each block by checking the clock at set points
• Flag and move on rather than stalling on one question
• Use scheduled breaks to eat, drink, stretch and reset
• Leave the previous block behind; do not replay questions between blocks

After the exam

• Note the date results are expected and avoid timing decisions that depend on early results
• Record what worked and what did not while it is fresh
• Share your result with your school or program only as required
• If the result is not what you hoped, read the score report, pause, and make a structured plan before a retake; check the current retake rules
• Take a few days to recover before the next phase
