build an error log that you actually review instead of a second textbook

On my first pass through practice questions, I started typing huge multi-paragraph notes into a spreadsheet for every missed question. Now the document is hundreds of rows long and I never have time to re-read it.

How do you keep an error log concise enough that you can actually review it every week?
